With zero care management fees, it allows you to allocate more of your Home Care Package to services, rather than to the management and coordination of your care.
If you select the Self-Directed Care management option, Community Interlink will support you by developing and annually reviewing a personalised Care Plan, designed to ensure the delivery of services that best meet your needs.
We will also provide management of any of your financial statements.
Self-Directed Care incurs no additional charge above your Home Care Package administration fee.
